2011-2012 Release Form  

If you are interested in registering your dancer for classes, please download the registration brochure and the release forms and bring them with you to registration. Please have the release forms  filled out when you come. If you have any questions concerning registration or class placement please contact us at centredance@aol.com or 814-238-8685

FALL REGISTRATION
Returning students (from 2010-2011 dance year):
 August 11
from 4:00 pm-6:30 pm
 
New students:
August 11 
7:00 pm-9:00 pm

Additional registrations will be accepted on August 13th
from 4:00 pm-6:00 pm

 Bring with you your completed release form and nonrefundable $10
registration fee per class (a maximum of $50 per student). This is NOT to
be deducted from your tuition. YOU MUST REGISTER IN PERSON, OR
HAVE SOMEONE REGISTER FOR YOU, AS WE WILL HAND OUT
IMPORTANT INFORMATION, SUCH AS STUDENT/PARENT HANDBOOK,
UPDATED DRESS CODE AND A PAYMENT COUPON BOOKLET.
If you must mail in your registration it will be processed after all “in person”
registrations are completed.

CLASSES ARE FILLED ON A FIRST-COME, FIRST-SERVED BASIS.

CLASS PLACEMENT
Class placement is determined by faculty assessment only.  
Returning students will find their class assignment highlighted
on their Fall registration brochure schedule of classes
(the class where we feel you will benefit most). 
To expedite the registration process all questions MUST be addressed
by phone prior to registration.  
Students are evaluated in each level during the first four
weeks of classes to ensure correct placement.
